Job summary

Qualifications

• Excellent communication skills (verbal and written).
• Experience working in a team environment.
• Computer skills required; prior EMR experience preferred.
• Ability to work flexible hours, including days, evenings, nights, holidays, and weekends and on-call.
• Demonstrated leadership and critical thinking skills.

Responsibilities

• Coordinate nursing care, including direct patient care, patient/family education and transitions of care.
• Support professional nursing practice across settings and continuum of care.
• Plan, implement, evaluate and communicate all phases of nursing care for assigned patients.
• Delegate patient care according to skill level, experience, patient acuity, and resources.
