Job summary

Valence-Care is seeking a Clinical Field Nurse Supervisor to act as a senior clinical resource for our nursing team across IDD service areas in Brooklyn, the Bronx, NYC, Queens and Westchester. You will support clinicians with complex cases, clinical decision-making, care planning and regulatory interpretation.

This role drives quality improvement, staff development and emergency preparedness, participates in SHP/QAPI activities, and leads field visits and performance evaluations to ensure high

Qualifications

• Graduate of an accredited nursing program with ADN or BSN.
• Current NYS license as Registered Professional Nurse.
• CHHA and IDD Homecare experience is required.
• Up-to-date knowledge of OASIS.
• Must have a vehicle to travel to patients.

Responsibilities

• Oversee regulatory requirements and interpretation of OASIS guidance and time points.
• Engage in quality improvement initiatives, staff development and emergency preparedness.

• Lead field visits and accompany new staff post orientation during surveys.
• Support ongoing education programs and supervise performance evaluations.

Job Position Summary:

The Clinical Field Nurse Supervisor will serve as a senior clinical resource for the nursing team across all IDD service areas, including Brooklyn, the Bronx, NYC, Queens, and Westchester. The position will assist clinicians with complex IDD cases, clinical decision-making, care planning, regulatory requirements, and interpretation of OASIS guidance and time points. The position will also participate in quality improvement initiatives, staff development, emergency preparedness, technology implementation, and agency readiness activities.

Essential Responsibilities:

1. Knowledge of OASIS guidance, interpretation and time points


2. Knowledgeable of CHHA Regulatory guidelines per Center for Medicare and Medicaid, New York State Department of Health (DOH) and Joint Commission (JC)


3. Assist with orientation of the new staff on Extended Health Care's policies and procedures


4. Accompany the new staff into the field post orientation, conduct random field visits and complete the annual performance evaluations and accompany the staff during survey visits


5. Knowledge of Strategic Healthcare Programs (SHP) reports and utilize and conduct for staff performance


6. Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new organization personnel.


7. Assists the Educator in the planning, implementation and evaluation of in-service and continuing education programs.


8. Reviews patient's clinical diagnosis, medications, procedures and clinical course.


9. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.


10. Participate in McKesson/Netsmart Software Upgrade, Testing and Staff Education.


11. Participate in Quality Assurance Performance Improvement (QAPI) activities


12. Participate in the Agency's Emergency Preparedness Plan


13. Complies with acceptable professional standards and practice.


14. Maintains general and ongoing clinical oversight of all IDD Field Nurses

Nurse Requirements:

• Graduate of an accredited school of nursing; ADN or BSN


• CHHA and IDD Homecare experience is required


• Up-to-date knowledge of OASIS


• Must have current NYS license and registration to practice as a Registered Professional Nurse in New York State


• Must have a vehicle to travel to patients


• Bi-Lingual preferred (but, not a requirement)
