Job summary

LHC Group seeks an RN Field Assessor in Grass Valley, CA to perform Long Term Care insurance assessments and collect claimant demographics, physician data, and medications. You will evaluate independence and cognition, then document findings in real time after a brief cognitive screen.

The role requires comfort with point‑of‑care technology on a laptop or tablet and strong clinical reasoning to determine home safety and daily living abilities for older adults.

Qualifications

• Current, unencumbered RN license in state of practice.
• Minimum of 2 years in home health or elder nursing.
• Willing to obtain professional liability insurance.
• Valid driver’s license and access to dependable transportation.

Responsibilities

• Complete Long Term Care assessments and gather demographics, physician information, current medication list.

As a Registered Nurse Field Assessor (FA) you would be completing Long Term Care insurance assessments. These hands‑off assessments require you to obtain claimant demographics, physician information, a current medication list, and evaluate the claimant’s functional level of independence.

Responsibilities

The assessment process also involves completing a brief cognitive screening and evaluating the claimant’s ability to complete ADL and IADL tasks. The ideal candidate will be technologically savvy and be able to use a point of care web application/system on a laptop or tablet, in real time for documentation purposes.

• Must demonstrate good clinical reasoning and judgement as it relates to assessing ADL performance, cognition and home safety.
• Must be comfortable with the use point of care technology when completing assessments.

Qualifications

• Current, unencumbered RN licensure in state of practice.
• Minimum of 2 years of home health experience or 2 years nursing experience in an equivalent area of practice serving the older adult population.
• Has, or be willing to obtain, a professional liability insurance policy.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
