Registered Nurse Emergency Department Transition to Practice Program Nights - New

Manages individualized, outcome-oriented nursing care through the use of the nursing process. Provides care, treatment, and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, in accordance with departmental and hospital policies and procedures.

Job Description

  1. Medication Administration
  2. a. Demonstrates knowledge of and follows SSH policies and procedures for administering, transcribing, and recording medications.
  3. b. Completes medication reconciliation process following SSH policy and procedure.
  4. c. Demonstrates proper procedure for the documentation of narcotic withdrawal, administration, verifies count, wastes per policy, and resolves narcotic discrepancy.

2. Plan of care/Documentation/Patient Family Centered Care/Patient Experience

  1. a. Develop, evaluate, and update individualized plan of care for each patient and document outcomes.
  2. b. Initiate admission assessment within 8 hours of admission, identify and document patient/family/significant other teaching needs upon admission and throughout hospitalization.
  3. c. Continue assessment/reassessment and identify care needs within established nursing practice.
  4. d. Document all patient care following departmental policies, standards, disease processes, and nurse-sensitive indicators.
  5. e. Assess/reassess and document patient's comfort level and response to interventions.
  6. f. Advocate for patient/family, seeking help when needed.
  7. g. Raise ethical concerns with the clinical team and seek resources for ethical decisions.
  8. h. Communicate daily plan via whiteboard, updating every shift.
  9. i. Facilitate discharge/transfer planning aiming for 12:00 pm discharge, communicating barriers.

Minimum Education - Preferred: Graduate of an accredited School of Nursing

Minimum Work Experience: Previous Acute Care RN experience preferred

Required Licenses/Certifications: RN - Registered Nurse; Additional certifications may be required depending on department (BLS, ACLS, PALS, etc.)
