• Assess and record patient status and needs on admission or at point of care.
• Organize health data so it is accurate, complete and accessible.
• Identify problems needing immediate attention.
• Document diagnoses to support planned outcomes and care.
• Initiate and update Problem List at admission.
• Develop and implement age-appropriate care plans.
• Provide patient/family education grounded in evidence-based practice.
• Coordinate discharge planning with patient/family and care team.
• Communicate progress and plan of care at rounds and meetings.
• Refer for continuity of care as needed.
