Provides education and guidance to families regarding individual children's growth and development, health resources, and carries out service coordinator functions as defined by Early Intervention Standards.

  1. Provides direct intervention services to children 0 - 3 and their families in natural settings.
  2. Carries out service coordination functions when appropriate as defined by the Early Intervention standards.
  3. Conducts evaluations and other assessments.
  4. Develops and implements Individual Family Service Plans.
  5. Assists in transition planning prior to discharge from the program.
  6. Referrals to outside agencies as needed.
  7. Maintains client records and other administrative duties according to DPH and Lipton Early Intervention policies.
  8. Adhere and complete all CHL’s protocols, policies, and procedures.
  9. Early Intervention services are provided both in-person and/or by Telehealth.
  10. Attends in-service education and professional meetings as required.
  11. Respects diverse views, approaches, and contributes to maintaining an environment of professionalism, tolerance, civility, and acceptance towards all employees, clients, families, and visitors.
  12. Maintains regular, reliable, and predictable attendance.
  13. Ability to work independently and contribute to the overall success of the team.
  14. Flexibility, excellent interpersonal skills, verbal, written, and communication skills.
  15. Maintains productivity standards according to the collective bargaining agreement.
  16. Demonstrates knowledge of medical issues to assist families regarding health concerns as they pertain to the development of their young children.
  17. Demonstrates knowledge of typical and atypical infant/toddler development in perceptual/fine motor, gross motor, cognition, language and communication, self-care and personal/social areas.
  18. Demonstrates the ability to support positive child and family development.
  19. Develops functional intervention techniques appropriate for infants and toddlers with atypical development.

Qualifications:

  1. RN - Registered by the Massachusetts Board of Registration in Nursing
  2. CPR, First Aid
  3. DPH’s CEIS Certification or eligible for certification
  4. Current NPI or apply for NPI (National Provider Identifier number)
  5. Current valid US-issued driver's license and ability to provide a registered and insured automobile for work-related purposes, including travel between work sites.
  6. BSN or higher from an accredited nursing program
  7. All candidates will be subject to a CORI review as outlined in the regulations set by the Massachusetts Executive Office of Health and Human Services.
