Decypher is seeking a Nurse Case Manager to coordinate comprehensive patient care, including assessments, treatment planning, discharge planning, and education, ensuring continuity of care and adherence to standards. Onsite at Wright-Patterson AFB, you will oversee CM/UM processes and support staff development.

Qualifications include an RN license, ADN or BSN, BLS certification, and 4 years of discharge planning or case management experience (2 years if BSN). U.S. citizenship is required.

Qualifications

• Education: Associate Degree in Nursing or Bachelor of Science in Nursing.
• Certifications: Basic Life Support (BLS) and additional certifications listed.
• License: Must hold a current, full, active, and unrestricted RN license.
• Experience: Four years discharge planning or case management with ADN; two years with BSN, or recent nursing work in last 36 months.

Responsabilités

• Case Management: implement and evaluate CM and UM processes; coordinate nursing and social work services.
• Training & Staff Development: provide monthly in-service training; support staff orientation; conduct nursing peer reviews.
• Patient Care & Treatment Planning: assess needs; develop care plans; educate patients and caregivers; coordinate resources.
• Discharge Planning & Coordination: identify CM candidates; plan and coordinate discharge; arrange referrals and follow-ups; track progress.
• Compliance & Monitoring: maintain documentation; ensure standards; monitor outcomes, length of stay, and discharge effectiveness.
• Administrative Responsibilities: develop procedures; manage caseloads; participate in committees; ensure EEO and safety compliance.

Job Summary

The Nurse Case Manager coordinates and manages comprehensive patient care services, including clinical assessments, individualized treatment planning, discharge planning, care coordination, and patient education to promote continuity of care and optimal health outcomes. The role ensures compliance with applicable clinical standards, regulatory requirements, and organizational policies while monitoring resource utilization, supporting staff development, and identifying opportunities to improve the quality and efficiency of patient care.

Required Qualifications

• Education:
  • Associate Degree in Nursing or
  • Bachelor of Science in Nursing
• Certifications:
  • Basic Life Support (BLS)

At least one (1) of the following certifications:

• National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC)
• Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC)
• American Nurses Credentialing Center: Nurse Case Manager (RN-NCM)
• National Academy of Certified Care Managers: Care Manager Certified (CMC)
• License:
  • Must hold a current, full, active, and unrestricted license to practice as a Registered Nurse.
• Experience:
  • For candidates with an Associate Degree in Nursing:
    • Minimum of four (4) years of experience in discharge planning or case management
  • For candidates with a Bachelor of Science in Nursing:
    • Minimum of two (2) years of full-time experience in discharge planning or case management OR
    • Full-time employment in a nursing field within the last 36 months is required

Key Responsibilities

• Case Management: Implement and evaluate case management (CM) and utilization management (UM) processes, identify improvement opportunities, and coordinate nursing and social work services.
• Training & Staff Development: Provide monthly in-service training, support staff orientation, share clinical expertise, and conduct quarterly nursing peer reviews as assigned.
• Patient Care & Treatment Planning: Assess patient needs, document clinical information, develop individualized care plans, provide patient and caregiver education, and coordinate community resources to ensure continuity of care.
• Discharge Planning & Coordination: Identify patients for case management, assess discharge readiness, develop and coordinate discharge plans, arrange referrals and follow‑up appointments, and monitor patients’ progress and care transitions.
• Compliance & Performance Monitoring: Maintain accurate documentation, comply with clinical, regulatory, and organizational standards, and monitor resource utilization, patient outcomes, length of stay, and discharge planning effectiveness.
• Administrative Responsibilities: Develop procedures, independently manage caseloads and projects, participate in facility committees, and comply with EEO, infection control, and safety requirements.

Work Environment & Schedule

Onsite, Wright Patterson AFB, Monday to Friday, between 0700 - 1630.

Physical requirements

The position requires the ability to sit for extended periods, stand and walk throughout the facility, operate a computer and telephone, and move safely between clinical and office environments. Occasional lifting, carrying, pushing, or pulling of supplies and equipment, as well as travel to community‑based healthcare settings, may be required depending on assigned duties.

Compensation & Benefits

Up to $ 44.11 per hour + H&W IF APPLICABLE
