• Develops and documents an individualized plan of care for each patient.

• Uses department software and Lotus Notes for communication and records.
• Adheres to privacy and security policies for patient information.

Responsibilities

• Administers, transcribes, and records medications per SSH policy.
• Completes medication reconciliation as required by SSH.
• Documents care, discharge planning, and patient education activities.
• Initiates admission assessment within 8 hours and documents teaching needs.
• Assesses and reassesses patient needs; communicates plans to the team.
• Ensures accurate charting of pain levels and responses to interventions.
• Facilitates discharge/transfer to meet 12:00pm goals and coordinates barriers.
