Signature HealthCARE is hiring an experienced RN for full-time day shifts at North Hardin Rehab & Wellness Center. You will provide direct nursing care, ensure patient safety, and mentor nursing staff in a collaborative team-focused environment.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physicians of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities and regulatory surveys as needed.
