Job summary

Community Connections Inc. is hiring a Registered Nurse for our Fall River, MA Day Center. You will oversee health-care services for individuals with disabilities, provide direct care and in-service training, and coordinate medical services with primary physicians.

Responsibilities include dispensing meds, conducting meds reviews, and maintaining health records. Requirements include MA RN license, 1+ year in community health, and a valid driver's license. Competitive benefits offered.

Qualifications

• Fully licensed with the Massachusetts Nursing Board.
• Bachelor's degree from a four-year college or university preferred.
• 1+ year experience in a community health setting.
• 1+ year experience in a training program for individuals with developmental disabilities.
• Valid driver's license and access to a motor vehicle.

Responsibilities

• Dispense and document consumer medications.
• Participate in individual case record reviews with the program team.
• Conduct medication reviews regularly.
• Review health concerns with the Clinical Team weekly.
• Monitor and record vital signs and weights.
• Contact physicians, families, guardians, and service coordinators as needed.
