The Clinical Nurse Coordinator (CNC) plays a key role in enhancing patient care coordination, quality, and communication across multidisciplinary teams. This position ensures seamless care delivery by collaborating with providers across oncology, radiation, surgery, and supportive services while tracking patient progress and serving as a primary point of contact for individuals with hematologic and solid tumor malignancies. The CNC provides comprehensive patient and family education, addressing treatment plans, side effects, and self-care needs throughout the cancer care continuum. Additionally, the role supports clinical practice by offering solution-oriented care, serving as a resource to staff, and delivering direct patient care as appropriate. The CNC assesses patient needs, facilitates communication among providers, participates in interdisciplinary care planning, and maintains a strong commitment to safety, quality, and patient-centered outcomes.
