The Smidt Heart Institute is home to a distinguished team of experts who are setting the bar for cardiovascular care in the United States.
The Institute has 16 centers and programs for diagnosing and treating heart and vascular conditions in patients of all ages.
Job SummaryCollaborate with the Congenital Heart Teams to provide seamless care for patients throughout their treatment journey. This includes establishing and revising plans of care, coordinating patient care activities, and managing clinical data from referral to follow-up.

Licenses:
Current, unrestricted California RN License, required
BLS from the American Heart Association or American Red Cross, required
Experience:
Minimum 2 years experience working with Congenital Heart Disease patients.
Case Management experience.
