The Smidt Heart Institute is home to a distinguished team of experts who are setting the bar for cardiovascular care in the United States.

The Institute has 16 centers and programs for diagnosing and treating heart and vascular conditions in patients of all ages.

Job Summary

Collaborate with the Congenital Heart Teams to provide seamless care for patients throughout their treatment journey. This includes establishing and revising plans of care, coordinating patient care activities, and managing clinical data from referral to follow-up.

Responsibilities
  • Establish and continuously update comprehensive diagnostic/treatment plans through interdisciplinary team input.
  • Coordinate patient care activities and clinical data management from initial referral to follow-up.
  • Develop, implement, and evaluate educational programs for patients, caregivers, and clinical staff.
  • Provide emotional and coping support to patients, families, and significant others, coordinating with support services to meet their needs.
  • Lead in developing standards of care for new therapies, following approved protocols for specialized nursing care.
  • Supervise referrals and transport volumes, striving to increase outpatient and inpatient referrals and patient participation in clinical and research programs.
  • Consult with physicians and healthcare providers on patient care and quality issues, supporting leadership and clinical teams with progress reports and quality measures.
  • Participate in quality improvement projects and recommend policy revisions based on current standards.
  • Assess educational needs and collaborate with nursing and medical personnel to evaluate educational program efficiency.
  • Coordinate patient programs, projects, and events with various departments and communities, serving as a resource for educational programs within Cedars-Sinai.
  • Develop and evaluate encouraging care plans based on patient and caregiver needs, preferences, beliefs, and values.
  • Promote and facilitate inter- and intra-departmental research.
  • Establish new transfer agreements and build referral relationships with local hospitals and key physician groups.
  • Coordinate care with other Nurse Navigators across divisions and departments.
  • Represent the Medical Center at regional and national conferences.

Qualifications

Qualifications -

Educational Requirements:

  • BSN (Bachelor of Science, Nursing), required
  • MSN (Master's Degree in Nursing), preferred
  • Sophisticated knowledge of congenital cardiac anatomy and physiology along with ability to identify cardiopulmonary pathophysiology dysfunctions including those involved with complex adult congenital heart disease, preferred

Licenses:

Current, unrestricted California RN License, required

BLS from the American Heart Association or American Red Cross, required

Experience:

Minimum 2 years experience working with Congenital Heart Disease patients.
Case Management experience.
