Job summary

Wood County is seeking a Registered Nurse for the Comprehensive Community Services (CCS CSP) program. The role supports treatment and recovery for individuals with mental health and substance use disorders within community settings.

The nurse provides medication monitoring, symptom management, and health assessments, working with a multidisciplinary team to deliver integrated services and coordinate care across settings.

Qualifications

• Bachelor’s Degree in Nursing required.
• Active Wisconsin RN license preferred or ability to obtain.
• Two years of nursing experience desirable in community health or related field.

Responsibilities

• Provide nursing services and medication monitoring for CCS CSP clients.
• Collaborate with psychiatrists, APNPs and other team members to develop care plans.
• Maintain accurate documentation and ensure compliance with state guidelines and policies.
• Support clients in achieving health goals through health promotion and disease prevention.
• Coordinate care and referrals, including transport to medical appointments.
