Registered Nurse Clinical Care Navigator

Responsibilities:

  • Facilitates communication between the patient, family members and healthcare providers to ensure patient satisfaction and quality of care.
  • Reviews primary diagnosis, co-morbidities, and clinical documentation in HCHB to validate or advise care center on applicable care needs.
  • Assists clinical staff with the identification of clinical findings which may indicate the need for additional services or an alternative care setting to maximize the patient's health condition.
  • Ensures appropriate referrals from home health, hospice, palliative and or personal care are coordinated effectively and efficiently.
  • Runs all applicable patient tracking, activity reports and documents in the system.
  • Assists with internal or external transfer of patients between care settings.
  • Works collaboratively with peers to support coordination efforts and patient care needs.
  • Complies with policies, procedures, and regulatory mandates.
  • Performs other duties as assigned.

Qualifications:

  • Current, unencumbered license to practice as a registered nurse specific to the state(s) you are assigned to work.
  • One year experience working in a hospice setting.
Preferred:
  • Previous home health experience.
  • Compact nursing license.
  • Previous call center and customer service experience.
