Job summary

HCA Houston Healthcare Medical Center is seeking an RN Case Manager for PRN opportunities to support patients and families through their care journey.

In this role, you will coordinate services, develop individualized treatment plans, and collaborate with physicians, nurses, and the interdisciplinary team to ensure seamless transitions and high-quality care across the continuum.

Qualifications

• Associates Degree required, Bachelor’s degree in Nursing preferred. Master’s Degree desirable or commensurate experience.
• Minimum of five (5) years recent acute care experience required.
• Minimum of two (2) years of utilization review/case management experience
• Minimum of one (1) year experience in discharge planning in an acute care setting highly desirable.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and needs.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction.

Job Summary And Qualifications

As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include
