Registered Nurse Case Manager (RNCM) - Hospice ($5,000 Sign On!)

Position Overview: We are seeking dedicated Registered Nurse Case Managers to join our hospice team. This full-time role offers a compensation range of $54-$60/hour, depending on experience, and coverage areas include North County San Diego (Inland and Coastal) and East County San Diego.

Key Responsibilities:
  1. Plan, organize, and direct hospice care with an emphasis on community health education and experience.
  2. Utilize community resources to meet the needs of patients and families within their homes and communities.
  3. Coordinate patient care for assigned caseloads.
  4. Communicate with physicians regarding patient needs and report changes in condition; obtain/receive physician orders as required.
  5. Educate patients and families/caregivers on self-care techniques.
  6. Provide medication, diet, and other instructions as ordered; recognize opportunities for health counseling.
  7. Collaborate with the interdisciplinary team and attend team meetings.
Qualifications:
  1. Graduate of an accredited nursing school with 1-2 years of recent acute care experience and 1-2 years in home care.
  2. Current licensure in the state and BLS CPR certification.
  3. Bachelor’s degree preferred, with hospice care experience.
  4. Ability to supervise hospice aides; management experience is not required.
  5. Excellent communication, problem-solving, and nursing skills.
  6. Physical ability to walk, stand, lift, and perform nursing duties.
  7. Valid driver’s license and insured vehicle.
  8. Holistic or alternative modality training preferred.
