Position: Field Registered Nurse Case Manager (RNCM)

Location: East LA / Pasadena / West Covina / Pomona areas

Employment Type: Full Time

Compensation: $50/hr + Mileage Reimbursement

Position Summary

We are seeking an experienced registered nurse to initiate and coordinate the hospice care plan, supervise CHHAs and LVNs, and provide skilled nursing hospice and palliative care services to patients at their residences. The RN will collaborate with the attending physician, Hospice Medical Director, and other Interdisciplinary Hospice Team members. Responsibilities include reviewing and updating care plans, performing prescribed medical treatments (including pain management and symptom control), conducting assessments, and providing education and supportive care to patients and caregivers. Proficiency with computers and electronic medical records is required.

Qualifications

• Graduate of an accredited school of nursing.
• Current California RN license. BSN preferred. Certification in Hospice and Palliative Nursing is a plus.
• Minimum of one-year experience as a professional nurse within the last three years or a Baccalaureate in Nursing from an NLN-accredited program.
• Valid driver's license, auto insurance, and reliable, well-maintained transportation.
• Spanish speaking ability is a plus but not required.
