POSITION SUMMARY
We are seeking an experienced registered nurse to initiate and coordinate hospice care plans, supervise CHHAs and LVNs, and provide skilled nursing hospice and palliative care services to patients at their residences. The RN will collaborate with attending physicians, the Hospice Medical Director, and other team members. Responsibilities include reviewing and updating care plans, performing prescribed medical treatments (including pain management and symptom control), conducting assessments, and providing education and support to patients and caregivers. Must be proficient with computers and comfortable using electronic medical records.
QUALIFICATIONS
- Graduate of an accredited nursing school.
- Current California RN license. BSN preferred. Certification in Hospice and Palliative Nursing is encouraged.
- Minimum of one year of professional nursing experience within the last three years or a Baccalaureate in Nursing from an NLN-accredited program.
- Valid driver's license, auto insurance, and reliable, well-maintained transportation.
- Spanish speaking ability is a plus but not required.
