Altra Home Health in Kokomo, IN seeks an experienced RN Case Manager to lead direct care and case management for a team serving clients in home health settings. The role complies with state Nurse Practice Act and agency policy.

The RN Case Manager will oversee RNs, LPNs, HHAs, and PCAs, with strong emphasis on communication, supervision, and regulatory knowledge. Travel is involved to cover on-site visits.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in state of practice (Baccalaureate preferred).
• Minimum of two years nursing experience; home health preferred.
• Ability to supervise and direct client care team.
• Strong written and verbal communication and interpersonal skills.
• Knowledge of home health regulatory and reimbursement requirements.
• Licensed driver with insured vehicle.

Responsibilities

• Reviews client referral information and responds to requests and inquiries.
• Performs initial and ongoing client assessments per agency policy.
• Directs, plans, and initiates appropriate action in home care situations.
• Manages/supervises a team of RNs, LPNs, HHAs, and PCAs.

Description:

Altra Home Health is actively searching for a Registered Nurse (RN) Case Manager. The RN Case Manager will provide direct care and case management for a team of clients in accordance with the state Nurse Practice Act and Agency policy. The RN Case Manager will be assigned and responsible for team members of RNs, LPNs, Home Health Aides, and Personal Care Assistants.

QUALIFICATIONS:

• Graduate of an accredited school of professional nursing.
• Current license as a Registered Nurse in the state(s) of practice. Baccalaureate degree preferred.
• Minimum of two (2) years of nursing experience. Home care experience preferred.
• Ability to supervise and direct members of client care team.
• Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
• Ability to deal tactfully with clients and the community.
• Demonstrated organizational and time management skills.
• Knowledge of home health regulatory and reimbursement requirements.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
