St. Croix Hospice seeks a Registered Nurse Case Manager to deliver 1:1 holistic home-based care, partnering with an interdisciplinary team to craft individualized plans, guide end-of-life conversations, and ensure comfortable, dignified patient experiences.

The role emphasizes autonomy and compassionate care in patients' homes, with on-call duties and opportunities for professional development and growth within a deeply caring team.

Qualifications

• RN licensure required.
• 1+ years of acute care experience; hospice/home health preferred.
• Complete assessments to determine hospice needs and develop care plans.
• Educate patients and caregivers on care techniques and medications.

Responsibilities

• Perform initial and ongoing comprehensive assessments for hospice needs.
• Develop individualized care plans with patient and family.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Coordinate on-call duties and care transitions.
