Position: Full-Time Registered Nurse Case Manager, Level 2

Territory: Natomas and surrounding areas

Role Overview:

As a Registered Nurse (RN), Level 2 Home Health Case Manager, you will utilize your clinical skills to create, implement, and execute care plans that promote wellness and prevent re-hospitalization.

You will oversee 2 nurses responsible for executing your care plans, conduct comprehensive assessments such as Start of Cares (SOCs), Evaluations, Resumption of Care, and discharge planning. You will also assist and educate patients and their families on wound care, blood draws, tube feedings, Tracheostomy care, Foley catheters, CHF management, and medication education in the home setting.

Additionally, you may be involved in other responsibilities as needed.
