Job summary

Hospice of the Midwest in Forest Lake, MN is seeking a registered nurse to join our team as an RN Case Manager. You will provide patient-centered hospice care, develop care plans, administer treatments, and collaborate with an interdisciplinary team to support patients and families.

We offer a Monday–Friday schedule from 8:00 AM to 5:00 PM, competitive salary range of USD 90,000–95,000 per year, and comprehensive benefits including health insurance, 401(k) with company match, and paid time off.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required; hospice or similar setting preferred.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
