Job summary

Hospice of the Midwest in Forest Lake, MN is seeking a registered nurse to join our team as an RN Case Manager. You will provide patient-centered hospice care in the Forest Lake area, collaborating with an interdisciplinary team and ensuring compassionate support for patients and families.

Requirements include state licensure, 1 year of nursing experience (3+ preferred), hospice knowledge and strong documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligning with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by the local branch.
