Registered Nurse Case Manager provides comprehensive home health services, coordinating care plans and collaborating with interdisciplinary teams to support patients and families in their homes. This full-time role ensures continuity of care and quality outcomes for the organization.
Conduct comprehensive patient assessments and develop individualized care plans
Coordinate and manage patient care across the continuum, ensuring quality and efficiency
Provide skilled nursing care in accordance with physician orders and established protocols
Monitor patient progress and adjust care plans as necessary
Educate patients and families on disease management, medications, and care techniques
Collaborate with physicians, therapists, social workers, and other healthcare professionals
Maintain accurate and timely documentation in compliance with regulatory standards
Ensure compliance with all state, federal, and agency guidelines
Active Registered Nurse (RN) license in the state of Idaho or compact license
Associate or Bachelor of Science in Nursing (ASN or BSN)
Minimum of 1-2 years of nursing experience; home health or case management experience preferred
Current Basic Life Support (BLS) certification
Valid driver's license and reliable transportation
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and manage time effectively
Experience with electronic medical records (EMR) systems
Knowledge of Medicare/Medicaid regulations in home health
