Job summary

Active RN license in Idaho or compact license is required, along with 1–2 years of nursing experience (home health preferred) and a current BLS certification. The role emphasizes strong assessment, planning, and coordination to achieve quality outcomes in a home-based setting.

Qualifications

• Active RN license in Idaho or compact license.
• 1–2 years of nursing experience; home health or case management preferred.
• Current Basic Life Support (BLS) certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Coordinate and manage patient care across the continuum, ensuring quality and efficiency.
• Provide skilled nursing care per physician orders and established protocols.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on disease management, medications, and care techniques.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals.
• Maintain accurate and timely documentation in compliance with regulatory standards.
• Ensure compliance with all state, federal, and agency guidelines.
