Job summary

Community Hospital is seeking a Full Time Registered Nurse Case Manager to coordinate care from admission through discharge and lead a multidisciplinary team.

The role requires Oklahoma RN licensure, 3+ years of hospital nursing, and utilization/case management experience. BLS within 30 days of hire is required. We offer comprehensive benefits and a supportive work environment.

Qualifications

• Current licensure with the Oklahoma State Board of Nursing.
• Minimum of three years hospital-based nursing practice with utilization/case management experience.
• BLS for Healthcare Providers required within 30 days of hire.

The RN Case Manager is responsible for comprehensive coordination of care and service of individual patients and/or patient populations, promoting effective utilization of resources, and assuming a leadership role in the multidisciplinary team to achieve patient and physician satisfaction and optimal outcomes. Must be able to work with all disciplines to meet patient needs from admission through discharge. Always works as a team member within the entire facility. Performs other duties as assigned.
