Position Purpose:

• RN Case Manager Hospice coordinates all interdisciplinary aspects of Hospice care in all settings- home base. Through assessment and reassessment functions, identifies patient/family needs and through the interdisciplinary processes, coordinates care provided. Evaluates patient /family response to care and coordinates needed changes to the Plan of Care. Oversees appropriate utilization of services and resources, appropriately authorizing care based on the plan of care.
• Specialty: Hospice and Palliative Care – in home care
• Location :Franklin County and six surrounding counties
• Hours of office:Day Shift: Monday – Friday 8:00a -4:30p, one weekend a month required
  
  

What You Will Do:

• Physical assessment including wound care and personal care
• Symptom management
• Working with Homecare Homebase – future will be EPIC
• Medication Management
• Management of 14 – 18 patients
  
  

Minimum Qualifications:

• Education: Graduate from school of nursing. Baccalaureate degree in nursing preferred
• Licensure / Certification: Current license to practice in the state of Ohio
• Experience: Minimum of two years work experience as a professional nurse, preferably one of the two years in an acute medical surgical, home health, oncology or hospice/palliative care setting.
• Ability to operate a motor vehicle required; must possess a valid drivers license and have access to a functional automobile during work hours.
