Position Summary

The Chronic Care Case Manager utilizes a collaborative process of assessment, planning, facilitation, and advocacy to meet Medicare patients’ health needs through communication and targeted intervention. The goal is to promote optimal, cost-effective outcomes in a clinical setting. This role involves assisting Medicare patients in managing their health plan benefits, coordinating necessary services, and removing barriers to engagement in appropriate care plans. The Case Manager works both independently and collaboratively within a multi-specialty healthcare environment.

This position may require rotating among several clinics to perform non-face-to-face encounters with the Medicare population in the specific area. All duties are performed in alignment with the Mission and Values of Hattiesburg Clinic.

Qualifications

• Graduate of an accredited nursing school; current Registered Nurse licensure in the state.
• Preferably at least three years of experience.
• Knowledge of EMR, medical management, policies, and procedures.
• Intermediate skills in word processing, spreadsheets, and database management.
• Excellent organizational, written, and oral communication skills.
