Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered Registered Nurse license.
• 1+ years experience in case management, preferably in healthcare/hospital.
• Exceptional interpersonal and communication skills to collaborate with professionals, patients and families.
• Proficiency in electronic health records (EHR) software.

Responsabilités

• Assess patients comprehensively including medical history, meds, treatment plans and psychosocial needs.
• Collaborate with healthcare providers, patients and families to develop and implement care plans.
• Monitor patient progress and adjust care plans as needed; communicate changes to the team.
• Serve as main point of contact between patient and providers for seamless care coordination.
• Maintain up-to-date records and case notes ensuring confidentiality and compliance.
