Job description

The Clinical Supervisor isresponsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor is responsible for ensuring that care andservicesaredelivered appropriatelyandforthe supervision of clinicalpersonnel.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Receives case referrals.Reviews available patient information related to case, includinghome visits, to determ in hospice/homecare needs.Assigns appropriate hospice personnel to case asneeded . Conferences with MEDICAL DIRECTOR regarding any questions aboutanindividual's eligibilityforservices.
• Reviews and evaluates each case through a variety of means such as home visits , conferences, record review and the services provided by clinicians ; discusses and verifies impressions, instructsand guidesclinicians topromotemore effectiveperformance anddeliveryof qualityhome careservices ; and is availableatalltimesduring operating hours toassistcliniciansasappropriate .
• Reviews patient's medicaldiagnosis, prognosis , medications , procedures andclinicalcourse .
• Assists clinicians in establishing immediate and long-termtherapeutic goals, in setting priorities , andindevelopingplanofcare.
• Attends caseconference meetings with hospice personnel tofacilitate coordinationofcareand discussion ofinterdisciplinary group involvement.
• Conductsquarterlyrecordreviewsand communicatesfindingsandrecommendations toClinical Director / Director of Patient CareServicesand hospice personnel.
• Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of newemployees.
• Assists Clinical Director/Director of Patient Care Services and other supervisory hospicepersonnel in the planning, implementation and evaluation of inservice and continuingeducation programs . Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range .
• Complies with accepted professional standards and principles.
• Assesses health needs of community and assists in the development qf plan(s)/ program(s)to meet needs . Participates in public relation and community activities that promote theorganization's role as an effective member of the hospice and health care delivery system . Promotescustomerserviceorientationtoallhospicepersonnel.
• Participatesactivelyinqualityassessmentperformanceimprovementteamsandactivities.
• Performs other duties and activities as delegated by the Clinical Director/Director of PatientCareServices .

POSITIONQUALIFICATIONS

• Registerednursewith current licensuretopracticeprofessionalnursing in thestate .
• Graduate of NLN accredited school of nursing with a BSN degree is preferred or a minimum of one(1)yearexperienceasaprofessional nursewithinthelastthree(3)years.
• Previous experience in hospice/home care setting and with two (2) years management or supervisory experience.Provenabilitytoworkwithinaninterdisciplinarysetting.
• Complieswithacceptedprofessionalstandardsandpractice.
• Has excellent observation, good nursing judgment and communicat i on skills . Understandshospicephilosophyandissuesofdeath/dying.
• Understands principles ofpain/symptommanagement.
• Must be a licensed driver with an automobile that is insured in accordance with state and / ororganizationrequirementsandisingoodworkingorder .
• Complies and maintainscurrent CPR certification.
• Demonstratesexcellentobservation , verbal andwrittencommunicationskills .
• Willing to hire H-1B Visa workers and assist with housing.
