Job summary

vTech Solution is seeking a Registered Nurse - Case Manager with acute care experience to provide skilled nursing care and manage discharge planning across hospital units, including Medical/Surgical and ED. The role requires WA state licensure, Epic EMR documentation, and proactive care coordination in a fast-paced environment.

The ideal candidate has at least 1 year in acute care, with diverse case management experience preferred. Weekend coverage is required on an every-other-weekend basis.

Qualifications

• Active Registered Nurse license in Washington State (WA).
• Minimum of 1 year acute care nursing experience.
• BLS certification.
• Experience with Epic EMR required.
• Strong discharge planning and care coordination skills.
• Ability to work effectively in a fast-paced hospital environment.

Job Summary:

The Registered Nurse - Case Manager with acute care experience will provide skilled nursing care and case management across multiple hospital units including Medical/Surgical, Emergency Department, and others. This role involves discharge planning, care coordination, and collaboration with interdisciplinary teams to ensure optimal patient outcomes. The nurse will utilize Epic EMR for documentation and must be licensed in Washington State. A minimum of one year of acute care nursing experience is required, with a strong preference for diverse case management experience.

Responsibilities:

• Provide skilled nursing care and case management in acute care settings.
• Perform patient assessments, discharge planning, and care coordination.
• Collaborate with interdisciplinary teams to develop and implement treatment plans.
• Document patient care and case management activities using Epic EMR.
• Float between hospital units including Medical/Surgical, ED, and others as needed.
• Ensure compliance with hospital policies and standards of care.
• Participate in weekend coverage on an every other weekend basis.

Required Skills & Certifications:

• Active Registered Nurse license in Washington State.

Preferred Skills & Certifications:

• Experience with AIDA EMR.
• Diverse case management experience including Med Surg, Long Length of Stay (LLOS), and Emergency Department.
• Previous travel nursing experience.

Special Considerations:

• Must float between hospital units including Colby and Pacific campuses.
• Work every other weekend is required.
• Level II Trauma Center environment.
• Scrub color restrictions: no reddish orange permitted.
• Pending Washington State RN license not accepted; active license required at start.
• Parking cost covered by facility.

Scheduling:

• Five 8-hour day shifts per week.
• Contract length of 13 weeks.
• Guaranteed 72 hours per two-week pay period.
