Job summary

Care 360 Hospice is seeking a Hospice Registered Nurse Case Manager in Toledo, Ohio. You will conduct patient assessments, travel to residences, and coordinate hospice care with the IDG to meet patient needs.

Required is an active RN license and BLS, with hospice or private-duty experience preferred. The role includes an on-call rotation and a Monday–Friday schedule with standard hours.

Qualifications

• Licensed Registered Nurse in Ohio.
• BLS certification is required.
• Hospice experience is preferred but not mandatory.

Responsibilities

• Conduct comprehensive patient assessments in homes and facilities.
• Travel to patient residences for visits and care planning.
• Develop and implement individualized care plans with the IDG.
• Educate patients and families on treatment plans and medications.
• Document clinical notes promptly and accurately.
• Participate in IDG meetings and case conferences.

Hospice Registered Nurse Case Manager

Salary: $80-89k/yr BOE

Schedule: M-F| 8:00AM-5:00PM +(on-call | 8-10 week rotation)

Location: Toledo, Ohio and surrounding area

Requirements of the RNCM role:

• Licensed Registered Nurse
• BLS Certified
• Comfortable working in both private homes and facility settings.
• Valid Drivers License and Auto insurance Coverage
• Reliable transportation
• A minimum of 1 year of Clinical experience
• Hospice Experience Preferred

About the RNCM role:

• Conduct comprehensive patient assessments, including vital signs and medical history review
• Travel to patients' residences to conduct visits
• Develop individualized care plans in collaboration with multidisciplinary teams
• Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
• Perform evaluations for hospice care along with informational visits as needed.
• Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.
• Educate patients and families on treatment plans, medication management, and health maintenance strategies
• Submit complete, accurate, and relevant clinical notes promptly.
• Participate in IDG and case conferences
• Performs on-call duties per policy and as needed
• Conducts an initial and ongoing comprehensive assessment of the patients
• Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.
• Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.
• Provide care plan to Hospice Aide and LPN as part of the IDG team.
• All other duties as assigned
