Job description

NOTE: We are recruiting for two Registered Nurses to work a total of 16 hours a week, to be scheduled within business hours of Monday through Friday 7:00 am to 5:00 pm.

The Tacoma Fire Department is currently seeking a motivated, analytical professional who is interested in challenging and varied work as a part-time Registered Nurse Case Manager for the EMS Prevention & Outreach Division. This classification is responsible for providing non-emergency case management including telephonic and in-person triage and assessment, development of patient-centered plans of care and coordinating care among an interdisciplinary team of medical and social service entities. The purpose of this role is to reduce the frequency of non-emergent 911 Emergency Medical Service (EMS) use by citizens who frequently call in the Tacoma Fire Jurisdiction.

The Registered Nurse (RN) Case Manager will work to identify the root cause of poor health management habits of citizens, motivate citizens to become advocates of their own health, and achieve quality health outcomes. The role entails performance of a full range of duties that demonstrates advanced, broad, and practical knowledge in a variety of processes, methods, techniques, and best practices within an assigned discipline. The RN Case Manager work involves planning and coordinating difficult assignments, the modification of established guidelines, and the initiative of new approaches, making daily work assignments, conducting work reviews, training staff in work procedures, and providing input to formal leadership. Duties will include community based and telephonic triage of assessments, development of patient-centered plans of care, care coordination among internal/external multidisciplinary team, and facilitation of safe care transitions in the community.

Job Responsibilities:

  • Performs case-finding, health teaching, health counseling and care coordination with providers in-person and with telecommunications and web-based programs; facilitates care among team members; coordinates meetings for timely clinical decision-making.
  • Responds to individuals seeking coordination of care; completes in-person assessments to identify health goals and needs; determines which community services are applicable and available; provides information, referrals and assistance.
  • Serves as a consistent contact for patients, families, and internal/external care providers; ensures the process of care is sustainable and provides a caring relationship to patients and families.
  • Identifies and resolves barriers to promote interdisciplinary collaboration and efficient patient care with appropriate internal/external providers.
  • Develops comprehensive patient care assessments, plans, and goals, using the principles of relationship-based care; communicates on regular basis with patients and families around the plan of care; teaches and coaches on disease management.
  • Coordinate meetings for timely, clinical decision-making and optimal handovers across the continuum of care.
  • May supervise nursing students participating in nursing rotations.
Minimum Education*
Graduation from an accredited school of nursing by the National League of Nursing or equivalent

Minimum Experience*
4 years of case management experience working with chronically ill populations suffering from multiple chronic diseases, behavioral health illnesses and social instability including three years of direct nursing care experience

Licensing, Certifications and Other Requirements

State of Washington RN License
State of Washington Driver's License
Pre-employment background check and drug screen
Care Coordination and Transition Management Certification preferred


*Equivalency: 1 year of experience = 1 year of education
  • Knowledge of current nursing principles and processes
  • Knowledge of anatomy, physiology, pathophysiology, epidemiology, physical assessment and basic pharmacology
  • Knowledge of healthcare technology, equipment, supplies, and materials used for medical treatment and care
  • Knowledge of current patient privacy laws, including HIPAA
  • Knowledge of Washington State law relating to nursing care (WAC 18.79), federal health care regulations, Practice guidelines, Patient Bill of Rights, and clinical practice guidelines for patient care; understanding of community standards
  • Ability to evaluate client needs and make referrals as appropriate
  • Strong communication skills, including oral and written, as well as the ability to actively listen

  • Ability to interpret and consolidate vague information from multiple sources

The Tacoma Fire Department CARES Program provides non-emergency services to assist community members beyond a traditional 911 emergency medical service (EMS) call. The CARES Program purpose is improving public safety and is a long-term strategic initiative for the department. CARES assists citizens navigate to appropriate levels of care and services so that can improve their health and remain safe in their home, not relying on 911-EMS for non-emergent medical needs.
