Job Description: RN Case Manager

Location: Tarzana, CA

Contract Length: 13 Weeks

Shift: Day, 5x8-Hour (08:00 - 16:30)

Position Overview: The RN Case Manager will be responsible for managing patient care in an acute care hospital setting. This role involves both Utilization Management (UM) and Care Coordination, with a focus on discharge planning. The RN Case Manager will collaborate with healthcare providers, patients, and families to ensure that the care plan is followed, and that patients are transitioned smoothly from hospital to home or other care settings.

Key Responsibilities:

• Conduct comprehensive assessments of patient needs and coordinate care plans.
• Manage the utilization of hospital resources through effective UM strategies.
• Develop and implement discharge plans, ensuring appropriate and timely discharge of patients.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Provide education and support to patients and their families regarding care plans and discharge instructions.
• Monitor and document patient progress, making adjustments to care plans as necessary.
  
  

Qualifications:

• Active RN license in the state of California.
• Minimum of 2 years of acute care hospital experience, with a strong background in both UM and Care Coordination.
• Experience in discharge planning within a hospital setting.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
  
  

Preferred Qualifications:

• Certification in Case Management (CCM or ACM).
• Experience with electronic medical records (EMR) systems.
  
  

Contract Details:

• 13-week contract with potential for extension.
