Qualifications

Responsibilities

• Perform care coordination and discharge planning for patients.
• Utilize InterQual Criteria for admission criteria in the Emergency Department.
• Participate in the development of treatment plans and family meetings.
• Work with interdisciplinary support including Social Services and Transportation.
• Arrange Durable Medical Equipment (DME), Skilled Nursing Facility (SNF), and ambulance transport independently.
• Manage patient diagnoses, including drug/alcohol abuse treatment, homelessness, mental health crises, and more.
• Document patient care using computerized systems.

Base pay range
