Job summary

A leading hospice provider in Lombard, IL is seeking a Registered Nurse Case Manager to deliver compassionate end-of-life care. The ideal candidate will hold an accredited nursing degree and have at least a year of recent acute care experience, preferably in hospice or home health. Responsibilities

• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.

The starting salary range for this role is $78,000–$106,000 annual base salary. Actual salary will be based on location, education, certifications, experience, and skillsets.
