Job description

Summary

Registered Nurse Case Managers are directly responsible to designated supervisors in their assigned areas. They oversee continuity of care, access, utilization management, clinical guideline implementation, monitoring, and performance improvement within their area. This role involves direct care of both inpatient and outpatient groups and requires collaboration within an interdisciplinary team.

Qualifications

Basic Requirements:

• Proficiency in English (per 38 U.S.C. 7403(f)).
• Graduate of an accredited nursing school (ACEN or CCNE).
• Eligibility for or possession of a valid, unrestricted nursing license in the U.S.
• Foreign graduates must have a current registration and may require degree validation.
• For Graduate Nurse Technicians: meeting education requirements and pursuing licensure within 120 days.

Preferred Experience: Outpatient clinic/primary care experience preferred. BSN preferred.

Duties

• Provide and supervise direct patient care, assessing, planning, implementing, and evaluating based on age-specific needs.
• Coordinate care focusing on patient transition, education, and self-management post-discharge.
• Administer medications and procedures following policies.
• Collaborate with interdisciplinary team members to influence care outcomes.
• Participate in quality improvement and customer service initiatives.
• Serve as a case manager across various clinics, cross-training as needed between Troy Bowling and Sousley Campuses.
