Are you a compassionate Registered Nurse Case Manager looking for an opportunity to make a difference in the lives of individuals with physical and developmental disabilities? At Merakey, we put heart and soul into everything we do.

We are seeking a Registered Nurse Case Manager to join our team at our programs in Bucks, Montgomery, and Philadelphia counties.

As a Registered Nurse at Merakey, you'll play a vital role in providing direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include clinical assessments, medication administration, and collaborating with a multidisciplinary team to ensure comprehensive care.

Essential Job Functions:

1. Travel to group homes, deliver medications, update MARS and Treatment logs, coordinate medical care, staff in-service on clients' care plans, ensure medical charts and care are in compliance with regulations. Attend meetings, complete quarterly nursing reports and medical charting in the database. Assist in new admissions and/or readmissions into group homes.
2. Perform nursing assessments on residents, as needed, to identify and address health issues promptly.
3. Collaborate with healthcare professionals and specialists to coordinate care, keeping the nurse manager informed.
4. Provide health education to residents and their families, including implementing self-medication programs as directed by the nurse manager.
5. Conduct in-service training for staff to ensure compliance with protocols and procedures, promoting excellence in care delivery.
6. Communicate health issues with the interdisciplinary team and actively participate in staff meetings.
7. Complete quarterly nursing reports.
8. Perform other duties as assigned to ensure smooth program operation.

Qualifications

• Graduation from an accredited RN program.
• Current RN license in the Commonwealth of PA.
• Preferably, at least one year of RN experience, with experience as a Case Manager or Visiting Nurse preferred.
• Genuine interest in working with individuals with disabilities.
• Possession of a valid PA driver's license and reliable transportation (may be required).
• Ability to perform professional nursing duties including assessment, treatment, case management, and computer literacy (Outlook experience preferred).
• Physical ability to exert up to 50 lbs occasionally, 20 lbs frequently, and 10 lbs continuously.
