Job summary

LiveWell Homecare Agency in Pennsylvania is seeking an experienced RN Case Manager to coordinate patient-centered care and optimize health outcomes in a remote-capable role.

You will assess, plan, implement, and monitor care, collaborate with families and healthcare teams, and maintain thorough documentation while advancing quality and fiscal performance.

Qualifications

• Minimum 2 years RN experience; case management experience preferred.
• BSN preferred; ACMA certification preferred.
• Knowledge of healthcare laws, regulations, and insurance policies is helpful.
• Ability to work independently and collaboratively within a multidisciplinary team.
