Job summary

Visiting Nurse Health System is seeking a Full-time Registered Nurse Case Manager for Hospice, serving Atlanta and Decatur. You will coordinate patient care in home settings, develop care plans, educate families, and ensure quality outcomes while adhering to utilization guidelines.

The ideal candidate has a Georgia RN license, CPR, and home health experience, with strong collaboration and patient education skills. Driver’s license required.

Qualifications

• Graduate from an accredited nursing program (Diploma, Associate, or BSN preferred).
• Hold current Georgia RN licensure and valid CPR certification.
• Possess a valid driver's license and reliable transportation.
• Experience in home health settings is preferred for PRN roles.
• Knowledge of Medicare/Medicaid regulations as applied to hospice/home health care.
• Demonstrated ability to collaborate with interdisciplinary teams.
• Strong time-management and independent decision-making in a home environment.
• Proficient with computers and basic HR/documentation systems.

Responsibilities

• Develop and implement individualized care plans with physician orders and interdisciplinary input.
• Modify and update care plans as patient conditions change and re-evaluate outcomes.
• Educate patients and families on disease processes, self-management, and preventing complications.
• Complete and submit OASIS, visit notes, and records within timeliness guidelines.
• Communicate patient status changes clearly with patients, physicians, and team members.
• Provide emotional support and connect with MSW and community resources as needed.
• Supervise LPNs and HHAs to ensure compliant, patient-centered care plans.
• Participate in SOC/ROC calls and weekly team conferences.
• Deliver high-quality care in the home per VNHS policies and standards.
• Maintain competencies through in-services and continuing education.
• Schedule visits efficiently and manage caseload to optimize productivity.
• Precept newly hired staff and coordinate with office staff regarding patient status.

Job Overview

Visiting Nurse Health System is hiring a Full-time/ Registered Nurse Case Managerfor Hospice, responsible for coordinating care for a caseload of patients and ensuring it meets quality standards and utilization guidelines. The RN Case Manager assesses, plans, implements, monitors, and evaluates comprehensive care plans to ensure optimal patient outcomes. This position demands a compassionate, skilled nurse who thrives in a home-based environment and values patient education and family engagement.

What You’ll Do:

• Develop and implement individualized care plans in accordance with physician orders by assessing patient needs and collaborating with the interdisciplinary team.

• Offer emotional support and connect patients/families with MSW and other community resources as needed.

• Demonstrate knowledge of Medicare, Medicaid, and funding source regulations as applied to hospice/home health care.
• Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams.
