Texas Health Rockwall is seeking a Registered Nurse, Care Transition Manager to join the ER-focused team. This full-time role offers days, Monday-Friday with some weekend coverage, and a potential move to a hybrid work arrangement as the department evolves.

You will contribute to patient transitions of care, partnering with nurses, physicians, and interdisciplinary teams to improve length of stay, readmission reduction, and patient experience, while benefiting from a supportive leadership

Work location: Texas Health Rockwall, with the potential to develop into a hybrid role.

Work hours:Full Time, Days working 40 hours weekly- Monday-Friday, with some weekend coverage at times, day shift, with flexibility as needed to support departmental operations. This position will mainly be for the ER.
