Job summary

You will conduct comprehensive nursing assessments, collaborate with a multidisciplinary team, provide wound care, IV therapy, and patient education, and supervise LPNs/LVNs and home health aides while maintaining thorough documentation.

Qualifications

• Must be licensed to practice as a registered nurse in the state of operation.
• At least 1 year of RN experience.
• CPR certification required.
• Must have a valid driver’s license and insured vehicle; ability to travel to multiple locations.

Responsibilities

• Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs.
• Collaborate with the interdisciplinary team to develop, implement, and evaluate care plans.
• Provide skilled nursing interventions including wound care, med management, IV therapy, and education.
• Deliver emotional support and education to patients and caregivers.
• Supervise LPNs/LVNs and home health aides; coordinate care and documentation.
• Respond to urgent needs, participate in admissions and on-call duties, adjust plans as needed.
• Ensure accurate clinical records and high-quality, compassionate care delivery.
