Job summary

St. Clair Hospital is seeking an RN Care Manager to ensure effective patient care delivery through collaboration with the multidisciplinary team. You will support discharge planning, care coordination, and utilization reviews across admission and post-acute care processes.

The role involves communicating with patients, physicians, and payors, coordinating referrals to community resources, and navigating EMR and web-based systems to optimize patient outcomes.

Qualifications

• Current licensure as a Registered Nurse in Pennsylvania.
• Two years of work experience as a Registered Nurse in an acute care setting.
• Highly effective communication skills.
• Strong negotiation skills and the ability to identify and address complex clinical and social issues with patient/family and members of the multidisciplinary care team.
• Ability to navigate electronic medical records and web based applications.

Working as a key member of the multidisciplinary team, the Care Manager ensures the effective delivery of patient care through a collaborative approach. The Care Manager interacts with all members of the care team on a daily basis to facilitate optimal discharge planning and care coordination. Additional duties include performing admission, outpatient, pre-admission, and continued stay utilization reviews, completing referrals to community resources and gathering and communicating relevant information to members of the care team including patients, physicians, and payors.

1. Current licensure as a Registered Nurse in Pennsylvania.
