Registered Nurse, Care Manager - Care Management

Position Overview

Located at Orlando Health Winnie Palmer Hospital for Women & Babies, opened in 2006, providing specialized care for women and newborns. Recognized for nursing excellence and high-quality patient care, the hospital welcomes nearly 14,000 babies annually and features one of the largest neonatal intensive care units in the country.

Role Summary

The Registered Nurse Care Manager promotes effective management of hospital resources from admission to discharge, collaborating with the clinical team to identify patients who will benefit from care coordination services, including assessing risk factors, clinical utilization management, and transition planning.

Shift

Varies - Full-time, Monday to Friday with rotating weekends and holidays.

Responsibilities
  1. Assess all patients within assigned population, including medical necessity screening and submission for Physician Advisor review.
  2. Coordinate care considering diagnosis, treatments, age, payment, and resources.
  3. Assign initial DRG, monitor LOS, and manage transition planning.
  4. Lead multidisciplinary care conferences and manage disputes.
  5. Make referrals, involve social work, and communicate with patients and families about care plans.
  6. Ensure compliance with regulatory standards and hospital policies.
  7. Provide education to patients, families, and staff.
  8. Develop collaborative relationships and monitor quality outcomes.
  9. Maintain documentation, participate in quality improvement, and stay updated with current literature.
  10. Perform additional duties as assigned.
Qualifications
  • Graduate of an approved nursing school.
  • Current Florida RN license and BLS certification.
  • Handle with Care certification within 90 days for Behavioral Health Unit.
  • Minimum three years of experience in care management or related fields.
