We offer
TheRegistered Nurse - Care Management plans, organizes, and provides nursing services to clinic patients in conjunction with clinic providers. This position works collaboratively with providers and other members of the health care team to improve the health of patients with chronic disease conditions and/or complex needs; educates patients and families to help them manage their health care needs; facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.
Education - Completion of a BSN, or an ADN from an accredited institution.
Experience - Two or more years of ambulatory nursing experience is highly preferred. BSN or three years with an ADN is preferred. Experience in areas such as public health, community health, or equivalent and experience in multicultural nursing or other social service area and commitment to community health care is preferred. Care management or chronic disease management experience is highly preferred.
Other Requirement(s) -
Active WA State Registered Nurse (RN) license required. Active Basic Life Support (BLS) for Healthcare Providers required. Active specialty certification in care management within twelve (12) months of hire required. Certified Diabetes Educator (CDE) preferred.
