Job description

The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities between two or more participants. The RNCC uses advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. The RNCC marshals and coordinates resources and patient care activities to support transition management through collaboration across the continuum, supporting patient and families' needs for follow-up and coordination of services across healthcare services and locations.

This job profile integrates the ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses with Interpretive Statements, the AAACN Scope & Standards of Practice for Professional Ambulatory Care Nursing, and the AAACN Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management, aligned with the UVA Nursing Professional Practice Model.

Care coordination and transition management require professional assessment, patient risk identification and stratification, and recognition of individual patient needs and preferences, including but not limited to the following responsibilities:

  1. Demonstrate the use of the UVA Professional Practice Model through nursing practice, quality achievement, lifelong learning, empowered leadership, innovation, and expert caring.
  2. Plan, coordinate, and prioritize patient care activities considering patients' unique needs and desired outcomes in collaboration with the inter-professional team, including consult recommendations and escalation as needed.
  3. Maintain safety and continuity of care through documentation, hand-off tools/processes, etc.
  4. Collaborate and advise patients, families, and caregivers in healthcare decisions, respecting their culture and values.
  5. Provide health education and coaching tailored to issues identified within treatment and service plans, following evidence-based care standards.
  6. Provide facilitative leadership promoting health equity across the continuum and among interdisciplinary teams to improve population health, patient experience, and reduce costs.
  7. Apply research and evidence-based practices to improve patient care across the lifespan and continuum.
  8. Ensure continuity and consistency of care across the continuum, including pre-visit coordination, post-clinic follow-up, and handoff between services, monitoring and facilitating transitions of care.
  9. Educate patients and families with chronic illness about evidence-based standards to empower self-management strategies.
  10. Identify support needs and develop action plans for patient-centered care planning and nursing process application.
  11. Contribute to problem-solving through communication and collaboration, evaluating treatment outcomes and tracking patient progress.
  12. Support medication management.
  13. Perform other duties as assigned.

Position Compensation Range: $41.26 - $54.52 Hourly

MINIMUM REQUIREMENTS:

  • Education: Bachelor of Science in Nursing from an accredited program.
  • Experience: 1 year of registered nursing experience required; 3 years preferred.
  • Specialty Board Certification strongly preferred.
  • License/Certification: RN license in Virginia; AHA BLS certification required.

PHYSICAL DEMANDS:

Standing for prolonged periods, frequent travel, bending/stooping, proficient communication, auditory and visual skills, attention to detail, ability to lift/push/pull 20-50 lbs, exposure to chemicals, blood, body fluids, and infectious diseases.
