Job description

The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient and family-centered care activities among multiple participants. The RNCC uses advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. The RNCC coordinates resources and care activities to support transition management through collaboration across the continuum, addressing patient and family needs for follow-up and service coordination across healthcare settings.

This job profile aligns with the ANA Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses, the AAACN Scope & Standards of Practice for Professional Ambulatory Care Nursing, and the UVA Nursing Professional Practice Model.

Care coordination and transition management require professional assessment, patient risk identification, and needs and preferences evaluation, including but not limited to:

  • Demonstrating the UVA Professional Practice Model through nursing practice, quality achievement, lifelong learning, leadership, innovation, and caring.
  • Planning, coordinating, and prioritizing patient care considering individual needs and outcomes in collaboration with the inter-professional team, including making consult recommendations and escalation when necessary.
  • Ensuring safety and continuity of care through documentation, hand-offs, and other methods.
  • Collaborating with and advising patients, families, and caregivers, respecting their cultural backgrounds and values.
  • Providing tailored health education and coaching based on evidence-based practices.
  • Promoting health equity across the continuum and among interdisciplinary teams to improve population health, patient experience, and reduce costs.
  • Applying research and evidence-based practices to improve patient care across the lifespan and continuum.
  • Leading efforts to ensure seamless care transitions, including pre-visit coordination, post-visit follow-up, and handoffs.
  • Educating patients with chronic illnesses on evidence-based self-management strategies.
  • Identifying support needs and developing care plans centered on patient needs.
  • Engaging in problem-solving through communication and collaboration, and evaluating treatment outcomes and progress.
  • Supporting medication management.
  • Performing other duties as assigned.

Position Compensation Range: $41.26 - $54.52 Hourly

MINIMUM REQUIREMENTS:

  • Education: Bachelor of Science in Nursing from an accredited program.
  • Experience: At least 1 year of RN experience required; 3 years preferred.
  • Certification: Specialty Board Certification strongly preferred.
  • License/Certification: RN license in Virginia; AHA BLS certification required.

PHYSICAL DEMANDS:

Requires standing, traveling, bending/stooping; good communication, auditory, and visual skills; ability to lift/push/pull 20-50 lbs; may be exposed to chemicals, blood, body fluids, and infectious diseases.
