Demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
Qualifications
Basic Requirements:
- English Language Proficiency: In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless proficient in basic written and spoken English.
- Education: Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE at the time the program was completed.
- Alternative pathways include: completion of a master's level bridge program in nursing, foreign nursing education with valid registration, or current unrestricted registration in the US.
- Licensure: Current, full, active, and unrestricted registration as a graduate professional nurse in the US.
- Graduate Nurse Technician (GNT) Exception: Candidates without licensure may be appointed temporarily while pursuing licensure, with provisions for extension.

Responsibilities
- The Care Coordinator addresses psychosocial, nursing, and medical needs through interdisciplinary collaboration.
- Coordinates care from diagnosis to treatment, manages laboratory and radiology results, and utilizes databases for population management.
- Evaluates care and outcomes, advocates for holistic care through education, and adapts to stressful situations effectively.
- Position requires visual acuity, keen hearing, clear speech, manual dexterity, and physical activity associated with nursing duties.
