Qualifications:

• Have an Associate’s Degree in Nursing and Licensed to practice as a Registered Nurse in the State of Michigan.
• One year of professional nursing experience.
• Strong clinical and communication skills.
• Ability to travel within the service area, provide valid Michigan driver’s license and auto insurance.
  
  

Duties and Responsibilities:

• Support the activities of the MI Choice Waiver program, providing community supports for nursing facility level of care individuals
• Independently perform intake and assessment, determine eligibility, develop care plans, and arrange services for program participants
• Provide case management, reassessment, and long-term monitoring of participant needs and provider performance
• Utilize registered nurse support coordination techniques and demonstrate clinical skills in geriatric care
• Maintain knowledge of local, state, and federal regulatory standards affecting the community program for senior citizens
• Work under general supervision, following established methods and procedures in the form of regulations, rules, policies, and oral instructions
• Authorize waiver services and coordinate and link services with formal and informal supports as well as arranged services
• Provide education and training to program participants and their families
