Primary Function/General Purpose of Position - The Care Coordination Team Lead/Supervisor provides clinical supervision and management of the Population Health team. This role will assume responsibility for daily operations. The Care Coordination Team Lead/Supervisor applies system population health standards to identify and assign patients with uncontrolled chronic conditions and/or rising risk indicators and uses critical thinking and leadership skills in managing the team to meet patients and their families’ needs.
This is a remote/work at home position. Hire must be able to work eastern time zone hours.

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient\'s status and interprets the appropriate information needed to identify each patient\'s requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
